Family Reunification Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at the Employment Opportunity & Training Center of NE PA. As a Family Reunification Case Manager, you will play a critical role in supporting families with children in out-of-home placement, with the ultimate goal of transitioning them to permanent placement with their parents.
Key Responsibilities- Act as the primary liaison with Office of Youth & Family Services (OYFS) caseworker(s), collaborating in the development of transition plans and placement/care recommendations.
- Support, coach, case manage, and monitor children and their families/caregivers from the referral phase through reunification and beyond, providing supervised parent/child visits as needed.
- Cultivate community resources for children and families/caregivers with complex needs, coordinating with OYFS, Outreach's team, outside counselors, and other service providers to address reunification challenges and resolve service gaps.
- Attend OYFS Family Team Conferences, Monthly Staffing, and other meetings as required.
- Prepare and maintain case records, reports, and other documentation, entering client data into the agency's data collection system.
- Master's or Bachelor's degree in Counseling, Psychology, or Human Services, with related experience.
- Proficiency in Microsoft Word, Excel, Internet software, and Database software.
- Crisis Prevention and Intervention training preferred; Social service/MH/BH and case management experience preferred.
- Regional travel required to visit and/or transport children in placement and their families; reliable transportation and a valid PA Driver's License & auto insurance are necessary.
- Spanish language proficiency is preferred.
