Position Summary:
The Family Reunification Support Specialist is a vital member of the Outreach team, tasked with providing intensive case management for families with children in out-of-home placements. The primary goal is to facilitate a successful transition back to permanent placement with their parents.
Key Responsibilities:
- Serve as the main point of contact with the Office of Youth & Family Services (OYFS) caseworkers; collaborate on the development of transition plans and placement recommendations.
- Support, coach, and manage cases for children and their families/caretakers throughout the reunification process, including supervised visits when necessary. Provide information, referrals, and secure supportive services and resources. Travel may be required.
- Continuously assess the goals and service needs of children and families/caregivers identified for intensive case management services, both pre- and post-reunification.
- Build community resources for families with complex needs, coordinating with OYFS, Outreach's team, external counselors, and other service providers to address reunification challenges and service gaps.
- Participate in OYFS Family Team Conferences, Monthly Staffing, and other required meetings.
- Maintain accurate case records, reports, and documentation; enter client data into the agency's data collection system.
- Perform additional duties as assigned.
Qualifications:
To excel in this role, candidates must meet the following requirements:
- Education/Experience: A Master's or Bachelor's degree in Counseling, Psychology, or Human Services, along with relevant experience.
- Computer Skills: Proficiency in Microsoft Word, Excel, Internet software, and database management.
- Certificates and Licenses: Preferred qualifications include Crisis Prevention and Intervention training, experience in social services/mental health/behavioral health, and case management. Reliable transportation and a valid driver's license are required. Spanish language proficiency is a plus.
Work Environment:
The work environment is typically moderate in noise level.
Physical Demands:
The role may require frequent lifting or moving of items weighing up to 20-25 pounds.
