Compensation Rate: $19.00 Hourly

Overview:

As a vital member of the Parent Child Programs, the Family Reunification Case Manager plays a crucial role in providing intensive case management services to families with children in out-of-home placements. The primary objective is to facilitate a smooth transition back to permanent placement with parents.

Key Responsibilities:

  • Serve as the main point of contact with the Office of Youth & Family Services (OYFS) caseworkers, collaborating on transition plans and care recommendations.
  • Support and guide families through the reunification process, including supervised visits and securing necessary resources and services.
  • Continuously evaluate the goals and service needs of families and children involved in intensive case management.
  • Develop community resources to assist families with complex needs, coordinating with various service providers.
  • Participate in OYFS Family Team Conferences and other required meetings.
  • Maintain accurate case records and documentation, ensuring data is entered into the agency's collection system.
  • Perform additional duties as assigned.

Qualifications:

To be successful in this role, candidates must meet the following criteria:

  • Education: A Master's or Bachelor's degree in Counseling, Psychology, or Human Services, along with relevant experience.
  • Technical Skills: Proficiency in Microsoft Word, Excel, and database management.
  • Certifications: Preferred training in Crisis Prevention and Intervention, along with experience in social services and case management.
  • Transportation: Reliable transportation and a valid driver's license are required.
  • Language Skills: Proficiency in Spanish is preferred.

Work Environment: The work setting typically maintains a moderate noise level, and physical demands may include lifting up to 20-25 pounds.
