Job Overview

GENERAL SUMMARY:

Under the guidance of the Community Home Supervisor, this role is dedicated to aiding residential clients in achieving full community integration; fostering their self-esteem and skill enhancement; and promoting their independence in daily living.

EDUCATION AND EXPERIENCE:

  • A high school diploma or GED is mandatory.
  • Preferred: One year of direct experience working with adults who have intellectual and developmental disabilities.

SPECIAL EMPLOYMENT REQUIREMENTS:

  • Minimum age of 18 is required.
  • A valid driver's license is essential.
  • Must be able to work in a drug-free environment.
  • A criminal background check is a prerequisite.

KEY DUTIES AND RESPONSIBILITIES:

ESSENTIAL FUNCTIONS:

  • ENSURE that every individual served is treated with dignity, respect, and self-determination, and has the right to be fully integrated into their community with necessary supports.
  • SUPPORT the implementation of Person-Centered/Individual Service Plans, including tracking outcomes, medical needs, and prescribed techniques. DOCUMENT individual progress and PROVIDE feedback for the development or revision of these plans.
  • ADDRESS the daily needs of individuals, including medication administration, personal hygiene, meal preparation, and personal care.
  • CREATE a safe and nurturing home-like atmosphere for individuals, performing household tasks such as light cleaning, laundry, and grocery shopping.
  • HELP individuals identify personal interests and establish community connections with appropriate supports.
  • EXECUTE consumer behavior support strategies.
  • ENABLE individuals to engage in a variety of meaningful leisure activities within inclusive community programs, allowing them to participate in their chosen activities.
  • PROVIDE transportation for community activities, employment, volunteer opportunities, medical appointments, and personal destinations.
  • ADHERE to agency vehicle policies.

ADDITIONAL RESPONSIBILITIES:

  • AID the Residential Supervisor in procuring necessary supplies.
  • REPORT any unusual incidents promptly to the Residential Supervisor or Program Specialist.
  • COMPILE required reports.
  • MAINTAIN communication with families and advocates of residential clients.
  • PARTICIPATE in program staff meetings and required training sessions.
  • PROVIDE first aid and necessary medical interventions during emergencies.
  • COLLABORATE with supervisors to guide students, trainees, and volunteers.
  • BE AVAILABLE for overtime in the assigned home or any other designated location when reasonable notice is given by a supervisor or program specialist.

PHYSICAL DEMANDS:

The individual must be capable of meeting the established physical requirements of the role, which may include lifting various weights, providing mobility assistance, performing restraint techniques, and administering CPR/First Aid as needed.
