Position Overview

Care Coordination Specialist (Full Time)

3-4 Day Work Week

**FULL-TIME** COMPREHENSIVE BENEFITS PACKAGE

Community Care Alliance is seeking dedicated individuals eager to contribute positively to the Rhode Island community. Our organization operates a 24/7 triage facility that offers assessment and referral services for adults facing mental health and/or substance use challenges. The Care Coordination Specialist plays a crucial role in welcoming clients to our program, supporting the clinical team during client visits, and facilitating connections throughout the referral process.

Key Responsibilities:

  • Assist clients with necessary paperwork.
  • Provide information about the services offered to clients, their families, and friends.
  • Ensure the comfort and safety of clients during their time at the facility.
  • Monitor facility security systems to maintain a safe environment.
  • Guide clients through safety protocols upon entering the facility.
  • Assist with transportation arrangements for clients as needed.
  • Prepare treatment areas to ensure they are clean and safe prior to client transitions.
  • Maintain accurate documentation and logs as required.
  • Engage in insurance verification processes.
  • Continuously assess program needs to identify areas for support.
  • Provide additional clinical and administrative assistance as necessary.

Preferred Qualifications:

  • Personable and empathetic approach to client interactions.
  • Ability to quickly establish rapport with clients.
  • Skilled in applying verbal de-escalation techniques with clients in distress.
  • Effective team player with strong communication skills.
  • Proactive and engaged in client and colleague interactions.
  • Capable of multitasking and prioritizing in dynamic environments.
  • Familiarity with electronic health record systems.
  • Multilingual candidates are encouraged to apply and will be compensated accordingly.

Experience and Education Requirements:

  • Minimum of an associate's degree in human services or equivalent experience.
  • Preferred 1-3 years of experience in human services or related fields.
