Key Responsibilities

Requirements
  • Graduate of a School of Nursing with current licensure to practice as a Registered Nurse in the State of Rhode Island.
  • Three years of clinical experience with recent experience in case management, community case management, patient navigation, or discharge planning.
  • Strong analytical and interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a collaborative approach to problem-solving.
  • Proficiency in the use of Microsoft Office software, including email and Outlook calendar.
Preferred Qualifications
  • Certification in Case Management by a nationally recognized organization.
  • Bachelor's Degree in Nursing or a related field.
  • Experience working in a hospital or healthcare setting, with a focus on case management or patient navigation.
Work Environment and Physical Requirements

The Case Manager will work in a general hospital environment, with occasional stressful conditions associated with patient care. The ability to make hospital rounds and navigate through various patient care areas is required. Visual acuity and finger dexterity are necessary for reviewing and carrying medical records, navigating through automated system screens, and typing on a computer terminal keyboard. Lifting of up to 10 lbs. may be necessary to transport items from one care unit to the next.

Supervisory Responsibility

The Case Manager will not have supervisory responsibility.
