Client Care Coordination Specialist (Full Time)

3-4 Day Work Week

**FULL-TIME** COMPREHENSIVE BENEFIT PACKAGE

Community Care Alliance is seeking dedicated individuals who are passionate about making a meaningful difference in the lives of individuals within the Rhode Island community. As a 24/7 triage facility, we provide critical assessment and referral services for adults facing mental health and/or substance use challenges. The Client Care Coordination Specialist plays a vital role in ensuring a welcoming environment for clients, supporting the clinical team during client visits, and facilitating essential connections throughout the referral process.

Key Responsibilities:

  • Assist clients with necessary paperwork.
  • Provide information about the services offered to clients, their families, and friends.
  • Ensure client comfort and safety during their time at the facility.
  • Monitor safety through facility surveillance and maintain a secure environment.
  • Guide clients through safety protocols upon entering the facility.
  • Assist with transportation arrangements to and from the facility as required.
  • Prepare treatment areas to ensure they are clean and safe prior to client transitions.
  • Maintain accurate documentation and logs as necessary.
  • Engage in insurance authorization processes.
  • Continuously assess program needs to identify areas for support.
  • Provide additional clinical and administrative assistance as needed.

Preferred Qualifications:

  • Compassionate and personable approach to client interactions.
  • Ability to quickly establish rapport with clients.
  • Skilled in applying verbal de-escalation techniques with clients in distress.
  • Effective communication and teamwork skills with other professionals.
  • Proactive and engaged in client and co-worker interactions.
  • Strong multitasking and prioritization abilities in dynamic environments.
  • Familiarity with electronic health record systems.
  • Multilingual candidates are encouraged to apply and will be compensated accordingly.

Educational Background:

  • Minimum of an associate's degree in human services or equivalent experience.
  • Preferred 1-3 years of experience in human services or related fields.
