Community Care Alliance is seeking dedicated individuals who are eager to contribute positively to the Rhode Island community. Our organization operates a 24/7 triage facility that provides essential assessment and referral services for adults facing mental health and/or substance use challenges. The Client Care Coordination Specialist plays a vital role in ensuring a welcoming environment for clients visiting our facility, supporting the clinical team throughout the client's experience, and facilitating effective connections during the referral process.
Key Responsibilities:
- Assist clients with necessary paperwork and documentation.
- Provide information about the services offered at Community Care Alliance to clients, their families, and friends.
- Ensure the comfort and safety of clients during their time at the facility.
- Monitor safety systems and maintain environmental security on the premises.
- Guide clients through safety protocols upon entering the facility.
- Assist with transportation arrangements to and from the facility as required.
- Prepare treatment areas to ensure they are safe and welcoming before client transitions.
- Maintain accurate documentation and logs as needed.
- Engage in the insurance authorization process.
- Continuously assess program needs to identify areas requiring support.
- Provide additional clinical and administrative assistance as necessary.
Desired Qualifications:
- Compassionate and personable approach to client interactions.
- Ability to quickly establish rapport with clients.
- Skilled in applying verbal de-escalation techniques with clients in distress.
- Strong teamwork skills and effective communication with other professionals.
- Proactive and engaged in client and colleague interactions.
- Ability to manage multiple tasks and prioritize effectively in a dynamic environment.
- Familiarity with various platforms, including electronic health records.
- Multilingual candidates are encouraged to apply and will be compensated accordingly.
Education and Experience:
- Minimum of an associate's degree in human services or equivalent experience.
- Preferred 1-3 years of experience in human services or related fields.
