Position Overview:

Key Responsibilities:
In this role, you will:

  • Engage in all patient consultations and follow-up visits, collaborating closely with physicians and advanced practice providers to assess patient needs.
  • Triaging patients to the appropriate care resources, ensuring effective communication through direct interactions and telephonic encounters.
  • Conduct thorough nursing assessments, including comprehensive patient histories and medication reconciliations.
  • Facilitate care conferences with a multi-disciplinary team to promote a collaborative approach to patient management.
  • Educate patients and their families about procedures, tailoring information based on individual assessments and expressed needs.
  • Communicate care plans and relevant information effectively among healthcare providers.
  • Complete necessary patient education and evaluations, including specific assessments related to cardiac procedures.
  • Serve as a resource for scheduling inquiries and problem-solving.
  • Compile and review diagnostic tests and laboratory results, coordinating with pre-surgical testing services as required.
  • Demonstrate a solid foundation in nursing theory and practice, showcasing expertise in a specialized area of care.
  • Manage a caseload of patients, working in partnership with the healthcare team to optimize health outcomes and coordinate care across various settings.
  • Lead initiatives in developing, validating, implementing, and evaluating standards and policies to enhance patient care delivery.
  • Support the healthcare team in achieving shared healthcare objectives through education and encouragement.
  • Provide expert consultation on clinical matters, fostering increased knowledge and skills among colleagues.
  • Participate in the education, clinical research, and mentorship of healthcare professionals.
  • Facilitate patient and family education in your area of expertise.
  • Complete mandatory training in safety, infection control, and basic life support annually.
  • Perform additional duties as assigned.

Qualifications:
To be considered for this position, candidates must possess:

  • Current licensure as a Registered Nurse in the relevant state.
  • A Bachelor’s degree in Nursing.
  • Certification in Basic Cardiac Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
  • A minimum of two years of relevant experience in cardiac care is preferred.

Work Environment:
This role is situated within a general hospital environment, which may involve occasional high-stress situations related to patient care.

Independent Action:
The Cardiac Care Nurse Coordinator will respond to individual patient care scenarios, demonstrating the knowledge and skills acquired through education and experience.
