GENERAL DESCRIPTION:

This position plays a vital role in supporting the community resources program by assisting the Liaison during facility visits and managing intricate cases. You will be part of a dedicated team, ensuring that all case documentation adheres to established standards.

ROLE AND RESPONSIBILITIES:

  • Assist the Community Transition Liaison in fulfilling their responsibilities effectively.
  • Accompany the Liaison on visits to nursing facilities as required.
  • Provide additional support for complex case management.
  • Collect necessary documentation to aid consumers in applying for public benefits.
  • Help consumers complete and submit housing applications.
  • Perform clerical tasks as directed by the Liaison or Supervisor, including data entry into designated cloud-based systems.
  • Ensure case documentation complies with standards set by the Executive Office of Elder Affairs.
  • Document all telephonic inquiries in the designated Call Log.
  • Respond to inquiries regarding ASNCM services and programs.
  • Verify necessary data elements for processing referrals.
  • Review recent journal notes prior to finalizing referrals.
  • Process referrals for Nursing Home Screenings and Adult Day Health Screenings.
  • Record and manage various service referrals, forwarding them to appropriate staff.
  • Maintain updated resources in the database annually.
  • Offer Options Counseling referrals when services are unavailable.
  • Conduct and analyze the results of the I&R QA Survey biannually.
  • Notify staff of scheduled office hours via calendar reminders.
  • Represent ASNCM at community health and resource fairs.
  • Perform additional duties as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

High School diploma or GED with at least one year of relevant experience or a strong interest in working with nursing facility residents, including older adults or individuals with disabilities transitioning to community settings.

A valid driver's license and a legally insured and registered vehicle are required, with documentation maintained in personnel records.
