Key Responsibilities:- Support individuals in learning, maintaining, or improving skills through their participation in various activities of everyday life, including in their homes and in the community to improve their level of independence.
- Implement support strategies to achieve the individual's personalized goals.
- Submit accurate and complete daily progress notes for each shift worked with the individuals.
- Bachelor's Degree or at least 2 years of comparable, community-based, life or work experience providing services to individuals with disabilities.
- Valid driver's license and existing insurance.
- Must be willing to drive individuals in personal cars throughout the community. We provide mileage reimbursement.
