Patient Care Coordinator

The Care Navigator at North Suffolk Community Services Inc. plays a crucial role in the Community Behavioral Health Center (CBHC) program. This position is responsible for overseeing case navigation, facilitating participation, and ensuring smooth transitions across all CBHC services. The Care Navigator provides education and support to individuals throughout their journey, acting as a vital community liaison and contributing to the daily operations of the programs.
Key Responsibilities
- Efficiently coordinate and execute referrals, ensuring timely and comprehensive communication with relevant parties, including:
- Participants' adherence to programs communicated to referral sources such as legal entities and child welfare services.
- Collaboration with external providers and community partners, including healthcare professionals and recovery support personnel.
- Engagement in outreach activities and follow-up appointments with program participants.
- Conduct real-time insurance verifications for clients.
- Demonstrate proficiency with eHana and EPIC systems (training will be provided), including:
- Reconciling cases based on call logs with state programs.
- Assist in managing non-clinical inquiries.
- Perform bed searches for various levels of care, including CCS, CBAT, and detox services as required.
- Under supervision, identify and report on risk and behavioral crisis situations to clinical staff.
- Meet with program participants to complete necessary documentation and administrative tasks.
- Ensure clinical documentation is accurately maintained in electronic health records and other databases.
- Maintain and update databases and spreadsheets for tracking information, communication, and client assignments as directed.
- Collaborate with clinical staff to develop and implement individualized action plans, monitor progress, and document outcomes as appropriate.
- Participate in team meetings and undertake additional responsibilities as assigned.
Qualifications
- Education and Experience:
- Bachelor's Degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or a related field.
- A minimum of two years of post-degree experience in a multidisciplinary treatment environment.
- Experience in group facilitation and working with individuals experiencing substance use disorders is essential.
