Position Summary:

In the role of Case Manager, you will collaborate with a primary care team at the JYP/BMC Clinic, delivering vital support services to patients in need. Your duties will encompass benefits assistance, housing support, and facilitating connections to primary care and behavioral health resources. This position presents a significant opportunity for professional growth for those with experience in case management.

Key Responsibilities:

  • Work alongside a multidisciplinary team to deliver comprehensive case management support.
  • Engage with a BHCHP primary care panel and provide adaptable case management services.
  • Participate in city-wide initiatives aimed at supporting homeless individuals.

Required Qualifications:

  • Ability to work autonomously while collaborating effectively with diverse teams.
  • Strong problem-solving capabilities and excellent communication skills.
  • Experience working with homeless populations is advantageous.
  • Proficiency in Microsoft Office and electronic medical records is essential.
  • Bilingual proficiency in Spanish or Haitian Creole is preferred.

Compensation and Benefits:

  • Starting hourly wage of $22.07, with potential increases based on experience.
