Key Responsibilities:
  • Efficiently coordinate and execute referrals, ensuring comprehensive communication with various stakeholders, including:
    • Program adherence updates to referral sources such as legal entities and child welfare agencies.
    • Collaboration with healthcare providers and community partners.
    • Engagement in outreach activities and follow-up with program participants.
  • Conduct real-time insurance verifications for clients.
  • Demonstrate proficiency with eHana and EPIC systems (training provided), including:
    • Reconciling cases from call logs with state programs.
  • Assist in managing non-clinical inquiries.
  • Perform bed searches for various levels of care as required.
  • Under supervision, identify and report risk and behavioral crisis needs to clinical staff.
  • Meet with program participants to complete necessary documentation and administrative tasks.
  • Maintain accurate clinical records in electronic health systems and other databases.
  • Support the maintenance of databases and spreadsheets for tracking information and client assignments.
  • Collaborate with clinical staff to develop and implement individualized action plans, monitor progress, and document appropriately.
  • Participate in team meetings and undertake additional responsibilities as assigned.

Qualifications:
  • Education and Experience:
    • Bachelor's Degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or a related field.
    • A minimum of two years of post-degree experience in a multidisciplinary treatment environment.
    • Experience in group facilitation and working with individuals facing substance use challenges is essential.
    • Experience level may vary based on educational qualifications.
