Position Summary:
This role is centered on administering both professional and nonprofessional nursing care services as per physician directives. The Patient Care Coordinator is responsible for:

  • Assessing, planning, implementing, and evaluating the care requirements of a specific patient group.
  • Providing nursing care and executing identified clinical support tasks to achieve the patient's care plan.
  • Delivering professional nursing services to patients, necessitating a valid nursing license.

Key Responsibilities:
This position entails the management of care for a designated patient population across the healthcare continuum. The Patient Care Coordinator is tasked with:

  • Ensuring well-coordinated experiences for patients and families by synchronizing care activities across multiple disciplines.
  • Negotiating with third-party payors to procure necessary services and resources for patients and families.
  • Facilitating quality and clinical customer service, addressing and resolving issues that negatively impact quality and cost.

Minimum Qualifications:
1. Associate's Degree in Nursing.
2. Basic Life Support (BLS) Certification.
3. Registered Nurse (RN) License.
4. A minimum of three (3) years of experience in Case Management.

Preferred Qualifications:
1. Bachelor's Degree in Nursing.

Duties and Responsibilities:
The following responsibilities outline the general nature of the work and are not exhaustive:

  • Conducting assessments for all new admissions to determine the need for Case Management services.
  • Participating in daily multidisciplinary rounds, reviewing patient care plans, and coordinating care.
  • Discussing treatment and discharge plans with the healthcare team and ensuring alignment with the patient's clinical course.
  • Identifying patients with complex psychosocial and financial needs and referring them to social work.
  • Maintaining positive relationships with referral agencies to enhance patient flow.

Physical Requirements:
1. Prolonged standing and walking.
2. Lifting and transferring patients.
3. Exposure to various patient care situations.

Skills & Abilities:
1. Knowledge of regulatory and payer requirements.
2. Proficiency in using designated screening tools.
3. Ability to provide care across the lifespan, considering cultural patterns and human development stages.
