Position Summary:


This role is responsible for facilitating the placement of program participants upon their release from designated institutions to community-based programs as identified by the receiving Placement Office.

Primary Duties:
  • Collaborate with Parole Officers to determine appropriate placements.
  • Work with the Transportation Coordinator to ensure transportation to program placements.
  • Assist the Community Service Representative in collaboration with the participant's Parole Officer to finalize placements.
  • Receive, track, and monitor referrals from Parole Officers.
  • Conduct assessments to ensure suitable placements based on individual needs.
  • Support the Community Service Representative in working with community-based providers to facilitate program reception.
  • Address concerns from Agents and communicate effectively with relevant authorities during non-placement situations.
  • Ensure participants have necessary medications and supplies prior to entering facilities.
  • Complete required reports as mandated by the relevant authorities.
  • Perform additional duties as assigned.

Qualifications:
Minimum Requirements:
  • High school diploma or equivalent.
  • Experience in case management and working with criminal justice populations is preferred.
  • Understanding of various models, including cognitive behavioral therapy and theories related to addiction and criminogenic risk factors.
  • Knowledge of the treatment continuum of care and the importance of transportation in participant retention.
  • Strong understanding of the transitional process between in-custody and community-based programs.
  • Excellent command of English and spelling.
  • Maturity to handle sensitive issues related to reentry substance abuse treatment services.
  • Ability to comply with program documentation requirements and professional standards.
  • Strong communication, organizational, and time management skills.
  • Capability to read and interpret documents such as regulations and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Skill in establishing rapport with participants and managing crisis situations.
  • Ability to facilitate participant engagement in treatment and recovery processes.
  • Possession of case management skills necessary for program implementation and ongoing assessment of participant needs.
  • Proficiency in using computers and relevant software.

Pay:
Compensation is set at USD $28.71 per hour.
