Position Overview:
This role is crucial in facilitating the transition of program participants upon their release from designated institutions to community-based programs as identified by the relevant placement office.
Key Responsibilities:
- Work collaboratively with Parole Officers to determine appropriate placements.
- Coordinate with the Transportation Coordinator to ensure participants are transported to their designated programs.
- Assist the Community Service Representative in working with participants and their Parole Officers to finalize placements.
- Receive, track, and monitor referrals from Parole Officers.
- Conduct assessments to place participants according to their individual needs.
- Collaborate with community-based providers to facilitate program reception.
- Address concerns from agents and maintain effective communication with relevant authorities during non-placement situations.
- Ensure participants have necessary medications and supplies before entering facilities.
- Complete required reports as mandated by relevant authorities.
- Perform additional duties as assigned.
Qualifications:
Minimum Requirements:
- High school diploma or equivalent.
- Preferred experience in case management and working with criminal justice populations.
- Understanding of various therapeutic models, including cognitive behavioral therapy and addiction theories.
- Knowledge of the California Treatment Initiative and the importance of transportation in participant retention.
- Extensive understanding of the transitional process between in-custody and community-based programs.
- Strong command of English language and spelling.
- Maturity to handle sensitive issues related to reentry services.
- Ability to comply with program documentation and ethical standards.
- Excellent communication, organizational, and time management skills.
- Ability to interpret legal documents and operating instructions.
- Capability to write routine reports and correspondence.
- Skill in establishing rapport with participants and managing crisis situations.
- Ability to oversee participants effectively and engage them in the recovery process.
- Possession of case management skills necessary for program implementation and assessment.
- Proficiency in using computers and relevant software.
