We are seeking a highly skilled and compassionate Clinical Program Coordinator II to join our team at Rebekah Children's Services. As a key member of our staff, you will play a vital role in providing exceptional care and support to children and families in need.
Key Responsibilities- Facilitate assessment, care planning, and coordination of services for Katie A Subclass members, ensuring that medically necessary services are accessed, coordinated, and delivered in a strength-based, individualized, youth/family-driven, and culturally and linguistically relevant manner.
- Establish and maintain collaborative relationships among children/youth, their families, and involved child-serving systems, supporting parents/caregivers in meeting their child/youth's needs.
- Organize and match care across providers and child-serving systems to allow children/youth to be served in their home community, ensuring seamless transitions and continuity of care.
- Participate in the gathering of information necessary for the creation of Coordinated Care Documents, such as CANS, SCC MH Assessment, and Treatment Plan, outcome measures, etc.
- Provide quality, culturally relevant services to children and their families in community-based settings and in-home services, coordinating the Child and Family Teams' work with the youth and their supports to identify their strengths, needs, and resources.
- Coordinate with system partners in the development and periodic revision of the Treatment Plan, monitoring progress to determine the effectiveness of interventions and determining the need for supplementary or supportive services.
- Develop transition plans to foster long-term stability, including the effective use of natural supports and community resources, and work collaboratively with other agency staff and community partners to ensure relevance, accessibility, and quality of service provided to children and their families.
- Document necessary information as required within the program in a timely manner, including youth documentation, report writing, and accurate data collection to assess progress and outcome.
- Master's degree in Marriage and Family Therapy, Social Work, Psychology, or a related field, with a license preferred (LCSW, LMFT, LPCC, Licensed Psychologist).
- One year of experience working with high-needs children and families, with experience in children's mental health settings emphasizing a family systems perspective preferred.
- Advanced skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication, with bilingual/bicultural (Spanish/English) skills required.
- Ability to work independently and as part of a team, with strong leadership and organizational skills, and the ability to complete assessments and develop and implement individualized treatment plans.
- Knowledge of and ability to access other community resources, with the ability to work with a balance of independence and creative use of supervision from the Clinical Program Manager.
