Case Management Specialist

POSITION SUMMARY:
All Together Recovery LLC (ATR) is dedicated to providing various levels of ASAM SUD/BHSO care, including recovery housing, educational programs, job training, and employment services, alongside family support in both for-profit and non-profit settings. The Targeted Case Manager (TCM) will manage a caseload of up to twenty-five clients, ensuring the delivery of high-quality case management, support, advocacy, and access to community resources. The TCM will conduct ongoing assessments and referrals for necessary services, adhering to state and federal regulations as well as company policies.
KEY RESPONSIBILITIES AND REQUIREMENTS:
- Possess a valid driver's license.
- Attend all required staff meetings.
- Demonstrate proficiency in ATR policies and procedures, educating clients accordingly.
- Exhibit servant leadership qualities.
- Maintain integrity in all professional interactions.
- Communicate effectively, responding promptly to all forms of correspondence.
- Ensure a minimum of four client contacts monthly, with at least two being face-to-face, or five for clients with dual diagnoses, while maintaining a caseload of twenty-five.
- Collaborate with clinicians to review treatment goals, develop crisis prevention plans, and conduct strength-based assessments with clients and their families, documenting as per state and office regulations.
- Complete contact notes within 48 hours of meetings.
- Ensure all legal documentation is secured prior to service delivery.
- Conduct quarterly reviews of treatment goals and adjust as necessary following team discussions.
- Stay informed about local resources available in clients' counties of residence.
- Understand clients' diagnoses and the associated challenges, providing necessary support and information to clients and their families.
- Collaborate with other service providers to coordinate treatment, ensuring integration across home, school, and community settings to achieve desired outcomes.
- Engage in continuous improvement initiatives for organizational services.
- Demonstrate ethical problem-solving and conflict resolution skills.
- Maintain a professional demeanor at all times.
- Facilitate interdisciplinary team meetings as required.
- Provide direct care to individuals, families, and groups, including resource linkage, service monitoring, advocacy, discharge planning, and follow-up support for families when applicable.
- Document, review, and update treatment plans as necessary.
- Ensure timely completion of all required contacts, with at least two being direct interactions.
- Submit all necessary documentation and maintain client records in compliance with state and federal regulations and company policies.
- Work effectively both independently and as part of a team.
- Follow through on all tasks to completion.
- Be flexible with work hours to accommodate client and family schedules.
- Reliable transportation may be required for travel.
Additional responsibilities may be assigned by the clinical program manager.
EDUCATIONAL BACKGROUND:
- Bachelor’s degree in a human services or behavioral science field, with at least one year of relevant post-educational experience in a human services environment.
- Successfully complete company-approved TCM training within six weeks of employment.
- Ability to thrive in a fast-paced, dynamic work environment.
