The successful candidate will oversee the entire patient care process, which includes assessing, planning, implementing, monitoring, and evaluating the necessary actions to meet clients' health and human service needs. Certification as a case manager is advantageous.
Key Responsibilities:- Deliver safe, timely, effective, efficient, equitable, and client-focused care
- Manage case assignments, develop service plans, monitor case progress, and determine case closure
- Assist clients in achieving wellness and independence
- Facilitate various care components, including case coordination and information sharing
- Advocate for patients by helping them make informed decisions regarding their clinical status and treatment options
- Build effective working relationships and collaborate with the medical team throughout the case management process
- Engage with patients to track their progress and ensure their satisfaction
- Document case information accurately, complete all necessary forms, and generate statistical reports
- Encourage quality and cost-effective interventions and outcomes
- Comply with professional standards as outlined by relevant protocols, rules, and regulations
- Demonstrated experience in case management, nursing, or related healthcare roles
- Strong understanding of case management principles, healthcare management, and reimbursement processes; knowledge of Medicaid is a plus
- Excellent communication skills
- Outstanding organizational and time management abilities
- Problem-solving skills with the capacity to handle multiple tasks
- Compassionate team player
- Current license, certification, or registration in a relevant field
- Bachelor's degree in a related healthcare discipline
