  • Facilitate various care components, including case coordination and information sharing
  • Advocate for patients by helping them make informed decisions regarding their clinical status and treatment options
  • Build effective working relationships and collaborate with the medical team throughout the case management process
  • Engage with patients to track their progress and ensure their satisfaction
  • Document case information accurately, complete all necessary forms, and generate statistical reports
  • Encourage quality and cost-effective interventions and outcomes
  • Comply with professional standards as outlined by relevant protocols, rules, and regulations
Qualifications:
  • Demonstrated experience in case management, nursing, or related healthcare roles
  • Strong understanding of case management principles, healthcare management, and reimbursement processes; knowledge of Medicaid is a plus
  • Excellent communication skills
  • Outstanding organizational and time management abilities
  • Problem-solving skills with the capacity to handle multiple tasks
  • Compassionate team player
  • Current license, certification, or registration in a relevant field
  • Bachelor's degree in a related healthcare discipline
