Client Support Specialist

Salary: $16-$18/hourly
POSITION SUMMARY:
All Together Recovery LLC (ATR) offers a variety of ASAM SUD/BHSO care services, including recovery housing, educational programs, job training, and employment assistance, as well as support for families in both for-profit and non-profit settings. The Targeted Case Manager (TCM) will manage a caseload of up to twenty-five clients, delivering comprehensive case management, advocacy, and access to community resources. The TCM will ensure that all services are in compliance with state and federal regulations, as well as company policies.
KEY RESPONSIBILITIES:
- Maintain a valid driver's license.
- Attend all required staff meetings.
- Understand and educate clients on ATR policies and procedures.
- Exhibit servant leadership qualities.
- Act with integrity in all professional interactions.
- Communicate effectively and respond promptly to all correspondence.
- Conduct a minimum of four client contacts monthly, including at least two face-to-face meetings, or five for clients with dual diagnoses.
- Review and assist in the development of treatment goals and crisis prevention plans, documenting as required.
- Complete contact notes within 48 hours of meetings.
- Ensure all legal documentation is secured prior to service delivery.
- Review treatment goals quarterly and adjust as necessary after team discussions.
- Stay informed about local resources relevant to clients' needs.
- Understand clients' diagnoses and provide necessary support to them and their families.
- Collaborate with other service providers to coordinate comprehensive treatment plans.
- Engage in continuous improvement initiatives for organizational services.
- Demonstrate ethical problem-solving and conflict resolution skills.
- Maintain a professional demeanor at all times.
- Facilitate interdisciplinary team meetings as required.
- Provide direct care to individuals and families, including resource linkage and discharge planning.
- Document and update treatment plans as needed.
- Ensure timely completion of all required client contacts.
- Maintain accurate client records in accordance with regulations.
- Work effectively both independently and as part of a team.
- Complete all assigned tasks diligently.
- Be flexible with work hours based on client needs.
- Possess reliable transportation for potential travel requirements.
Additional responsibilities may be assigned by the clinical program manager.
EDUCATIONAL REQUIREMENTS:
- Bachelor's degree in a human services or behavioral science field, with at least one year of relevant experience.
- Complete company-approved TCM training within six weeks of employment.
- Ability to thrive in a fast-paced, dynamic work environment.
