Job Summary:

The Unit Clinical Care Coordinator is a key member of the healthcare team responsible for managing patient flow efficiency within the unit. This role involves coordinating patient care activities, collaborating with the Hub and Clinical Care Coordinators to ensure appropriate patient placement, level of care, and patient progression.

Key Responsibilities:
  • Coordinate patient care activities to ensure efficient patient flow and effective patient outcomes.
  • Designate unit bed reservation priorities based on patient care needs and nurse staffing ratios.
  • Identify and remove unit barriers that may impede patient flow.
  • Assess patients' clinical presentation against Milliman Care Guidelines to determine admission status, working DRG, and target LOS.
  • Collaborate with Clinical Documentation Improvement Specialist (CDI Nurse) to validate admission orders and level of care.
  • Coordinate with Utilization Review (UR) Nurse to align payor and status requirements with patient's clinical progression.
  • Consult and educate medical and nursing staff regarding patient status, placement, payor requirements, and utilization of resources.
  • Assess patient's discharge needs and develop a comprehensive discharge plan that addresses patient needs, barriers, and readmission risk.
  • Collaborate with physicians to ensure patient progression and discharge needs are identified and a plan is developed to address barriers.
  • Lead the clinical care team in daily SNAP huddle to identify patient progression and discharge barriers.
  • Manage patient progression and discharge in Care Advance software and communicate with the Hub to ensure timely, ongoing communication of patient status.
  • Coordinate plan of care in collaboration with clinicians to ensure appropriate sequencing of care/interventions.
  • Communicate plan of care and discharge plan to patient and/or their family in coordination with the patient's primary care team.
  • Oversee and ensure care team's timely compliance with patient's preprocedural requirements for service department diagnostics and procedures.
  • Create and manage unit staffing and bed assignment for current and next shift.
  • Serve as a resource on patient status, progression, and care coordination for the unit Associates.
Requirements:
  • Minimum of three (3) years recent acute care nursing experience.
  • Bachelor of Science in Nursing is required.
  • Able to work independently, managing time, multiple priorities, and resources to achieve goals.
  • Able to maintain a calm demeanor and command during time of crisis management.
  • Able to assimilate information quickly to produce sound decisions and recognize situations that require immediate intervention.
  • Able to articulate information and ideas clearly through both written and verbal communication.
Preferred Qualifications:
  • One (1) year supervisory experience.
  • Utilization review experience.
License and/or Certifications:
  • Valid RN License from Virginia or reciprocal compact state required.
  • AHA BLS Provider CPR required at hire.
