As a key member of our organization, the Hospice Nursing Care Coordinator plays a vital role in delivering compassionate care to patients and their families. This position involves providing direct nursing services and collaborating with a multidisciplinary team to ensure a seamless care experience.
Key Responsibilities:
- Develop and implement individualized patient care plans, adjusting as necessary to meet evolving needs.
- Conduct thorough health assessments to inform nursing diagnoses.
- Administer prescribed medications and treatments in accordance with established care plans.
- Provide guidance and support to patients and their families in addressing nursing and related needs.
- Engage with physicians and participate in interdisciplinary meetings to address the comprehensive needs of patients and their caregivers.
- Ensure adherence to all applicable state and federal regulations.
- Graduation from an accredited nursing program with active licensure in the state of practice.
- A minimum of 1 year of nursing experience is required; 3 or more years is preferred.
- Experience in hospice care or a similar environment is advantageous.
- Understanding of the hospice care philosophy is essential.
