Position Overview: The Clinical Care Unit Coordinator (Unit C3) plays a pivotal role in overseeing the efficiency of patient flow at the unit level. This includes ensuring optimal patient placement, determining patient status, assigning Diagnosis-Related Groups (DRG) and Target Length of Stay (TLOS), and facilitating timely patient progression while managing staffing to enhance system efficiency.

Key Responsibilities:

  • Act as a central point of contact, providing leadership in unit progression and liaising between emergency and surgical services, clinical teams, and ancillary departments to meet patient care requirements effectively.
  • Collaborate with healthcare team members to enhance patient throughput, leading to improved patient-centered outcomes and organizational performance regarding length of stay.
  • Coordinate patient care activities with the Hub and Clinical Care Coordinators (C3s) to ensure appropriate patient placement and progression at the unit level.
  • Assess patient care needs and nurse staffing ratios to prioritize unit bed reservations, communicating these priorities to the Hub C3.
  • Identify and address barriers that may hinder patient flow within the department or across the system.
  • Evaluate patients' clinical conditions against Milliman Care Guidelines (MCG) to determine admission status, validate DRG, and ensure compliance with regulatory requirements.
  • Consult with and educate medical and nursing staff on patient status, placement, payer requirements, and resource utilization.
  • Assess discharge needs and develop comprehensive discharge plans that address patient requirements and potential readmission risks.
  • Lead daily huddles with the clinical care team to identify progression and discharge barriers, escalating unresolved issues to senior leadership.
  • Manage patient progression and discharge activities using Care Advance software, ensuring timely communication of patient status with the Hub.
  • Coordinate care plans with relevant clinicians to ensure appropriate sequencing of interventions and adherence to clinical pathways.
  • Communicate care and discharge plans to patients and their families in collaboration with the primary care team.
  • Oversee compliance with pre-procedural requirements for diagnostics and procedures, ensuring effective communication of any service event issues.
  • Create and manage unit staffing and bed assignments for current and upcoming shifts, serving as a resource for unit associates regarding patient status and care coordination.

Qualifications:

  • A minimum of three (3) years of recent experience in acute care nursing.
  • A Bachelor of Science in Nursing is required; RNs with an Associate's Degree must enroll in a BSN program within one year and complete it within five years.
  • Ability to quickly assimilate information for sound decision-making and recognize situations requiring immediate intervention.
  • Certification in nursing, case management, or utilization review is preferred.
