Position Overview:

The Clinical Care Unit Manager (CCUM) is tasked with overseeing the efficiency of patient flow within the unit by ensuring optimal patient placement, evaluating status, assigning Diagnosis-Related Groups (DRG) and Target Length of Stay (TLOS), and facilitating timely patient transitions. This role acts as a vital link between emergency and surgical services, clinical teams, and support departments to guarantee that patient care requirements are fulfilled and treatment plans are executed promptly.

Key Responsibilities:

  • Collaborates with the Hub and Clinical Care Coordinators to manage patient care activities, ensuring suitable patient placement and flow within the unit.
  • Prioritizes unit bed reservations based on patient care needs and nursing staff ratios, effectively communicating these priorities to the Hub.
  • Identifies and addresses obstacles within the unit that may hinder patient flow.
  • Evaluates patients' clinical conditions against established guidelines to determine admission status and validate orders.
  • Reviews patients in observation and outpatient statuses to assess the need for status changes.
  • Works with Utilization Review Nurses to align payer requirements with patient clinical progress.
  • Provides guidance and education to medical and nursing staff regarding patient status and resource utilization.
  • Develops comprehensive discharge plans that address patient needs and potential readmission risks.
  • Collaborates with physicians to ensure that patient progression and discharge needs are met.
  • Leads daily team huddles to identify and resolve patient progression barriers.
  • Manages patient progression and discharge processes using Care Advance software, ensuring effective communication with the Hub.
  • Coordinates care plans with relevant clinicians to ensure appropriate interventions are initiated.
  • Communicates care and discharge plans to patients and their families in collaboration with the primary care team.
  • Ensures compliance with pre-procedural requirements for diagnostics and procedures.
  • Oversees unit staffing and bed assignments, collaborating with management for staffing needs.
  • Acts as a resource for unit staff regarding patient status and care coordination.
  • Performs additional duties as required.

Qualifications:

Required:

  • A minimum of three (3) years of recent experience in acute care nursing.
  • A Bachelor of Science in Nursing is mandatory. RNs with an Associate's Degree must enroll in a BSN program within one year and complete it within five years.
  • Ability to work independently while managing multiple priorities effectively.
  • Capacity to maintain composure during crisis situations.
  • Quick assimilation of information to make sound decisions and recognize urgent situations.
  • Strong written and verbal communication skills.

Preferred:

  • At least one (1) year of supervisory experience is strongly preferred.
  • Experience in utilization review is advantageous.

Licenses and Certifications:

Required:

  • A valid RN License from Virginia or a reciprocal compact state is required.
  • AHA BLS Provider CPR certification is required upon hire.

Preferred:

  • Certification in nursing, case management, or utilization review is preferred.
