About the Role

We are seeking a compassionate and reliable Care Manager to join our team at Magnolia Home Care Llc. As a Care Manager, you will play a vital role in enhancing the quality of life of seniors, veterans, and individuals with disabilities by providing personalized care and support.

Key Responsibilities
  • Personal Care: Assist clients with personal hygiene needs, dressing, and grooming.
  • Mobility and Exercise: Assist clients with mobility, walking, and physical therapy exercises to maintain their independence.
  • Meal Preparation: Prepare nutritious meals and snacks to meet clients' dietary needs.
  • Housekeeping: Perform light housekeeping activities to maintain a clean and safe living environment.
  • Medication Reminders: Assist clients with medication reminders and management.
  • Companionship: Provide emotional support and companionship to clients, helping to alleviate feelings of loneliness and isolation.
  • Errands and Shopping: Assist clients with errands and shopping, ensuring they have access to essential items.
Requirements
  • Certification: Valid certification as a Nursing Assistant (CNA) or Personal Care Attendant (PCA).
  • Driver's License: Must possess a valid Virginia driver's license and have access to a reliable vehicle.
  • Background Check: Must have a clean criminal background check and drug screening.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
