Position Overview:
This role is essential in facilitating the transition of individuals back into society from correctional facilities or community supervision programs. The Community Reintegration Coordinator will work closely with clients to ensure they receive the necessary support and resources during their reintegration journey.

Key Responsibilities:
• Organize and lead orientation sessions for individuals interested in the Reintegration Program.
• Perform comprehensive assessments to evaluate the needs and risks of participants.
• Establish and manage Recovery Support Teams composed of both formal and informal support systems.
• Coordinate meetings for Recovery Support Teams to create a tailored Single Coordinated Care Plan (SCCP) that aligns with the individual’s strengths, needs, and preferences, aiming for measurable outcomes within a specified timeframe.
• Maintain accurate documentation of the SCCP, progress notes, crisis plans, and outcome data in a timely manner.
• Oversee the quality of services provided through the Recovery Support Team and adjust the SCCP as necessary to meet established guidelines, advocating for the consumer’s best interests.
• Provide or arrange transportation as needed, demonstrating flexibility to meet clients at various locations.
• Manage a caseload of clients, ensuring regular face-to-face interactions and participation in Recovery Support Team meetings, as well as other collaborative activities.
• Conduct interviews for required data collection instruments at intake, six months post-intake, and upon program exit.
• Deliver training sessions on the Wraparound philosophy and approach, guiding consumers through the AODA Wraparound process.
• Maintain ongoing contact with clients in diverse community settings.

Additional Responsibilities:
1. Assist in educating clients about their treatment options.
2. Participate in Recovery Support meetings as needed.
3. Attend training sessions to enhance facilitation skills.
4. Collaborate with various stakeholders involved in the consumer’s care, including probation and parole officers, social services, and legal representatives.
5. Support clients in accessing AODA treatment and recovery services, which may include mental health support, housing assistance, employment resources, and educational services.

Qualifications:

  • Bachelor's Degree in Criminal Justice, Social Work, or a related field.
  • A minimum of three years of relevant experience.
  • Familiarity with the Wraparound Philosophy is advantageous.
  • Valid driver's license and access to a vehicle are required.
  • Proficient computer skills, including typing at least 35 words per minute.
  • Ability to meet security clearance standards as required.
