Position Overview:

This role is responsible for coordinating services for individuals transitioning back into society from correctional facilities or those involved with community supervision. The position may also assist individuals from the broader community.

Key Responsibilities:

  • Facilitate monthly orientation sessions for participants.
  • Provide ongoing peer support and counseling to individuals currently incarcerated or reintegrating into the community.
  • Document participant interactions in the appropriate database.
  • Connect individuals with tailored community resources, vocational training, educational programs, and mental health services.
  • Offer continuous support through individualized mentoring and coaching to help reduce recidivism rates.
  • Collaborate with the Liaison to conduct orientation sessions for interested individuals.
  • Establish Recovery Support Teams to create a robust support network.
  • Engage in Recovery Support Team meetings to formulate a Single Coordinated Care Plan (SCCP) that aligns with the participant's strengths, needs, and preferences, ensuring measurable outcomes are achieved every month.
  • Coordinate transportation for participants as necessary and be adaptable to travel for client meetings and agency collaborations.
  • Manage a caseload of participants, ensuring regular face-to-face interactions, attending Recovery Support Team meetings, and participating in necessary administrative meetings.
  • Maintain ongoing contact with participants across various community settings.

Additional Responsibilities:

  • Assist in educating participants about the treatment process.
  • Engage in Recovery Support meetings as needed.
  • Attend training sessions to enhance facilitation skills.
  • Collaborate with relevant stakeholders including probation and parole officers, social services, and legal representatives.
  • Perform other duties as assigned by the Program Director.
  • Support participants in accessing treatment and recovery services, including mental health support, housing, employment, and educational services as identified.

Qualifications:

Skills and Abilities:

  • Strong organizational skills with a keen attention to detail.
  • Exceptional communication and writing abilities.
  • Experience working with diverse populations.
  • Understanding of substance abuse and addiction issues is essential.
  • Familiarity with the criminal justice system and community resources is preferred.
  • Experience in conducting interviews and facilitating meetings is advantageous.
  • Proficient computer skills are required.

Minimum Requirements:

  • Bachelor's Degree in Criminal Justice, Social Work, or a related field is mandatory.
  • A minimum of three years of relevant experience.
  • Knowledge of the Wraparound Philosophy is a plus.
  • Must possess a valid driver's license, automobile insurance, and access to a vehicle.
  • Ability to operate a computer and type at least 35 words per minute.
  • Must meet security clearance standards set by relevant authorities.

Work Environment:
Work is conducted both in an office setting and in the field, with approximately half of the time spent in the community. Flexibility in scheduling is required to accommodate the needs of participants.

Physical Requirements:
Position involves sitting for extended periods, traveling in a vehicle, and requires effective communication skills. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.
