We are seeking a highly skilled and compassionate Care Coordinator to join our team at Wisconsin Community Services. As a Care Coordinator, you will play a critical role in providing intensive and individualized care coordination for children and adolescents under the Wraparound Practice Model.
Key Responsibilities- Maintain a caseload of approximately 8 children and families, providing regular visits, phone contact, and support as needed.
- Assemble a Child and Family Team within two weeks of enrollment, identifying family members, natural supports, and service providers.
- Develop and implement individualized Plans of Care, including comprehensive 24-hour Crisis Plans, in collaboration with the Child and Family Team.
- Monitor the provision and quality of services, advocating for children and families across various settings, including home, educational, court, and community settings.
- Attend all scheduled court hearings as a representative of Wraparound Milwaukee, supporting children and families throughout the court process.
- Seek community resources, modify Service Authorization Requests (SARs) as needed, and provide or arrange for transportation for youth and families.
- Provide or secure support and crisis/emergency services, completing necessary paperwork in a strength-based manner.
- Maintain accurate information online, ensuring up-to-date family demographic information and Child and Family Team Member contact details.
- Bachelor's degree in a relevant area of education or human services, with a minimum of one year of experience providing mental health services.
- Completion of 85 hours of mandatory training in Wraparound philosophy and policies, as well as attendance at a Family Orientation within six months of hire date.
- Working knowledge of computer applications, including Windows and Microsoft Word.
- Ability to display cultural competence, responding respectfully and effectively to individuals from diverse backgrounds.
- Knowledge and skills to work with children, young adults, and families, including valid driver's license, automobile, and insurance.
- Adhere to agency and Code of Conduct policies, maintaining a respectful demeanor and presentation at all times.
- Conduct self in an ethical manner, maintaining professional relationships with program staff, other WCS staff, youth, families, and external agencies.
- Demonstrate commitment to agency values and mission, including sensitivity towards cultural, ethnic, and disability issues.
The work environment is primarily busy office settings, managing caseloads of children, young adults, and families. Some outreach is done in the community and families' homes, with occasional evening and weekend work required.
