Position Overview:

The Support and Service Coordinator plays a vital role in the CLTS Waiver Program, which is part of Wisconsin's Home and Community Based Services (HCBS) Medicaid Waiver initiatives. This program is designed to provide essential Medicaid funding for services that enhance the inclusion and access of eligible youth in their communities. Participants typically include young individuals aged 0-22 who have developmental, severe emotional, or physical challenges and meet the necessary functional and financial criteria for Medicaid.

Key Responsibilities:

  • Coordinate and facilitate comprehensive services tailored to the developmental, emotional, and physical needs of enrolled youth.
  • Deliver family-centered, trauma-informed, and culturally sensitive services through regular contact, ensuring engagement with families at least every three months.
  • Promote community inclusion and access for youth by providing services in various community settings.
  • Assist families in navigating program eligibility requirements and maintaining necessary documentation.
  • Conduct assessments and reassessments of the child's health, safety, and functional abilities.
  • Identify at-risk youth and ensure they receive appropriate care coordination, collaborating with other agencies as needed.
  • Provide crisis intervention and resolution for critical incidents.
  • Fulfill mandated reporting duties regarding the mental health and safety of youth to relevant authorities.
  • Develop and implement individualized service plans that cater to the unique needs of each family.
  • Authorize and manage the delivery of services under the CLTS Program.
  • Advocate for families, helping them access necessary resources and support independently.
  • Facilitate transitional support for youth entering or exiting the CLTS Program.
  • Complete all required training and maintain accurate documentation of services provided.
  • Adhere to all policies and procedures established by the organization.

Additional Responsibilities:

  • Participate in ongoing training and team meetings to enhance service delivery.
  • Support colleagues by providing coverage for families as needed.
  • Perform other related duties as required to fulfill job responsibilities.

Qualifications:

Successful candidates will possess:

  • A bachelor's degree in a relevant field and at least one year of experience working with individuals with disabilities.
  • Completion of all mandatory training related to the CLTS Program.
  • Proficiency in computer applications, particularly Microsoft Word and Excel.
  • The ability to provide trauma-informed, family-centered, and culturally competent services.
  • Strong interpersonal skills to effectively engage with youth and families.
  • A valid driver's license and reliable transportation.
  • Compliance with all employee requirements, including background checks.

Work Environment:

This position primarily involves working in an office setting while also requiring community outreach. Flexibility in scheduling is necessary, as some duties may occur during evenings or weekends.

Physical Requirements:

Staff may need to engage in physical activities while interacting with youth and may be required to drive as part of their duties.
