Position Overview: This role is dedicated to facilitating pre and post-release support for individuals transitioning back into society from correctional facilities or under community supervision. The position may also extend services to individuals in the broader community.
Key Responsibilities:
- Conduct monthly orientation sessions for participants.
- Provide weekly peer counseling and support to individuals currently incarcerated or reintegrating into the community.
- Document interactions and progress in the designated database.
- Connect participants with tailored community resources, including job training, educational programs, and mental health services.
- Offer ongoing mentorship and coaching to participants to encourage lower rates of reoffending.
- Collaborate with the Liaison to facilitate orientation sessions for interested individuals.
- Establish Recovery Support Teams to create a robust support network.
- Engage in Recovery Support Team meetings to formulate a Comprehensive Care Plan based on individual strengths and needs, ensuring regular reviews.
- Provide or arrange transportation for participants as necessary, demonstrating flexibility in meeting locations.
- Manage a caseload of clients, ensuring regular face-to-face interactions and participation in relevant meetings.
- Maintain consistent contact with clients across various community settings.
Additional Responsibilities:
- Educate and involve clients in their treatment journey.
- Participate in Recovery Support meetings as needed.
- Attend training sessions to enhance facilitation skills.
- Collaborate with other professionals involved in the client's care, including probation officers, social services, and legal representatives.
- Perform other duties as assigned by the Program Director.
- Assist clients in accessing treatment and recovery services, including housing and employment support.
Qualifications:
- Strong organizational skills with a keen attention to detail.
- Exceptional communication and writing abilities.
- Experience working with diverse populations.
- Understanding of substance abuse and addiction issues is essential.
- Familiarity with the criminal justice system and community resources is preferred.
- Experience in interviewing and facilitating meetings is advantageous.
- Proficient computer skills are required.
Minimum Requirements:
- Bachelor's Degree in Criminal Justice, Social Work, or Human Services is mandatory.
- A minimum of three years of relevant experience.
- Knowledge of the Wraparound Philosophy is a plus.
- A valid driver's license and access to a vehicle are required.
- Ability to type at least 35 words per minute.
- Must meet security clearance standards set by relevant authorities.
Work Environment: This position involves both office work and fieldwork, with approximately half of the time spent in the community. Flexibility in scheduling is necessary to accommodate client needs.
Physical Requirements: The role requires sitting for extended periods, traveling in a vehicle, and the ability to communicate effectively. Reasonable accommodations may be made for individuals with disabilities.
