LOCATION: Remote - must reside in or near Buncombe County, North Carolina.

GENERAL OVERVIEW:

The Healthcare Coordination Specialist is tasked with delivering proactive interventions and orchestrating care for eligible members of Vaya Health. This role is essential in ensuring that individuals receive suitable assessments and services tailored to their needs. The Specialist collaborates with members and their care teams to pinpoint and address inappropriate care levels or service gaps through comprehensive assessments, multidisciplinary care planning, and coordination of necessary services across various health domains, including mental health, substance use, intellectual/developmental disabilities, and physical health.

KEY RESPONSIBILITIES:

  • Utilization of Vaya's Care Management software platform for effective service delivery.
  • Engagement and outreach to members to foster connections and support.
  • Adherence to HIPAA regulations, ensuring confidentiality and proper handling of sensitive information.
  • Conducting Health Risk Assessments (HRA) to evaluate members' overall health, encompassing social determinants, mental health history, and resource accessibility.
  • Participation in interdisciplinary team meetings to develop comprehensive care plans.
  • Facilitating transitional care management for members moving between different levels of care.

ESSENTIAL FUNCTIONS:

  • Conducting thorough clinical assessments to identify members' needs and develop person-centered care plans.
  • Linking members with appropriate services and supports across all health domains.
  • Collaborating with care teams to ensure the delivery of integrated care and addressing any barriers to service access.
  • Monitoring and documenting care management activities to ensure compliance with all relevant standards and regulations.
  • Providing education and resources to members and their families to promote informed decision-making.

QUALIFICATIONS:

A Master's degree in a relevant field such as health, psychology, sociology, or social work is required. Candidates must also possess valid licensure in their respective fields. Experience working with individuals facing behavioral health challenges or requiring long-term services and supports is preferred.
