Overview

Good Seed Community Development Corporation is seeking a highly skilled and compassionate Lead Resident Care Case Manager to join our team. As a key member of our organization, you will play a vital role in providing comprehensive case management services to our clients, with a focus on supporting the unique needs of youth (age 16-25).

Responsibilities

  • Case Management
    • Develop and implement individualized service plans to address the complex needs of clients.
    • Provide crisis intervention and connect clients to essential resources, including health, mental health, employment, and housing services.
    • Monitor and evaluate client progress, making adjustments to service plans as needed.
  • Leadership and Teamwork
    • Oversee all services and operational activities, ensuring a professional and welcoming environment for clients, guests, vendors, and colleagues.
    • Manage and evaluate the performance of clients, providing guidance and support to achieve their goals.
    • Develop and maintain strong relationships with staff and subordinates, promoting a culture of collaboration and respect.
  • Administrative and Reporting
    • Maintain accurate and detailed documentation of client services, outcomes, and other relevant data.
    • Participate in client- and program-related meetings and training, staying up-to-date on best practices and industry developments.
  • Facility Management
    • Ensure the facility is clean, safe, and free from potential hazards, adhering to strict standards and protocols.
    • Enforce house rules and policies, promoting a positive and respectful environment for all.

Requirements

  • 4-year degree in human services or a comparable combination of education and work experience in a social service setting.
  • Working knowledge of case management and planning techniques, with experience working with homeless individuals and youth.
  • Valid driver's license and a clean driving record, with the ability to provide own transportation for work-related travel.
  • Excellent written and verbal communication skills, with the ability to work effectively in crises and de-escalate volatile situations.
  • Proficient in Microsoft Word and Excel, with a genuine interest in helping youth and a patient, inquisitive, and resourceful approach to problem-solving.
