We are seeking a highly skilled and compassionate On-Site Case Manager to join our team at Special Service for Groups, Inc- HOPICS. As a key member of our staff, you will be responsible for providing critical support services to individuals and families experiencing homelessness.
Key Responsibilities- Case Management: Develop and implement individualized case plans to address the unique needs of each client, ensuring their successful transition to permanent housing.
- Eligibility and Service Coordination: Conduct thorough assessments to determine client eligibility for our services, and coordinate with internal and external resources to provide comprehensive support.
- Triaging and Housing Placement: Work closely with clients to identify and secure suitable housing options, and provide ongoing support to ensure successful placement and retention.
- Documentation and Reporting: Maintain accurate and up-to-date records of client interactions, progress, and outcomes, and prepare regular reports to inform program development and evaluation.
- Collaboration and Communication: Foster strong relationships with internal stakeholders, community partners, and external agencies to ensure seamless service delivery and coordination.
- Education: Associate's degree in social services or a related field, or equivalent experience.
- Experience: Minimum 2 years of experience working with homeless populations, with a focus on case management and housing stability.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
- Knowledge: Working knowledge of Microsoft Office, HMIS, and other relevant software applications.
This position requires the ability to work in a fast-paced, dynamic environment, with frequent travel to various locations and occasional evening and weekend work.
