Job Summary

We are seeking a compassionate and experienced Case Manager to join our team at LHH Recruitment Solutions. As a Case Manager, you will play a critical role in providing personalized support to individuals and families experiencing homelessness.

Key Responsibilities
  • Client Intake and Assessment: Conduct thorough assessments to understand clients' needs for housing stability and develop individualized service plans.
  • Resource Coordination: Assist clients in accessing essential services, including emergency shelters, transitional housing, financial assistance, job training, and healthcare.
  • Case Management: Maintain regular contact with clients to monitor progress and work on adjustments to service plans.
  • Documentation: Maintain accurate and confidential records of client interactions.
Requirements
  • Experience: At least 1-2 years of experience in case management, social services, or a related field.
  • Skills: Strong interpersonal and communication skills, with the ability to work effectively with vulnerable populations.
  • Empathy: Demonstrated compassion and a non-judgmental approach to working with clients.
  • Technology: Proficiency with Microsoft Office Suite and experience using case management software or databases.
