Reports to: Manager of Nurse Case Management Services

Salary: $132,653.70 Annually

Status: Full-time; Exempt; Regular

Probation: 180 days

Role Summary: Under the guidance of the Manager of Nurse Case Management Services, the Healthcare Case Manager will deliver medical care management and comprehensive support services to patients. This role ensures that case management services adhere to industry standards and meet all contractual obligations. The ideal candidate must be a Registered Nurse and will collaborate with team members to implement, monitor, and uphold program guidelines.

Key Responsibilities:

  1. Collaborate with healthcare professionals and case management staff to develop and execute patient care plans for chronic disease management.
  2. Act as a vital member of the multidisciplinary Primary Care Team, ensuring compliance with protocols for all clinic programs.
  3. Provide oversight and support to Nurse Case Managers.
  4. Make independent and significant decisions regarding patient care.
  5. Conduct outreach to ensure clients are informed about available services.
  6. Work with staff to create and maintain an effective Utilization Management Program.
  7. Coordinate specialty referrals and assess patient eligibility for referral authorization.
  8. Ensure continuity of care and follow-up for clients.
  9. Serve as a liaison between specialist consultants and Health Services.
  10. Develop educational materials for clients and their families.
  11. Maintain clear communication through case presentations and written documentation.
  12. Oversee coordination with external agencies for services such as home health and hospice care.
  13. Monitor specialist appointments and reconcile funding and billing requirements.
  14. Provide skilled nursing care, including administering medications and treatments.
  15. Conduct assessments in accordance with regulatory guidelines.
  16. Assist in staff training and development.
  17. Participate in staff meetings and case conferences.
  18. Comply with all applicable regulations.
  19. Triage patients as necessary.
  20. Perform additional duties as assigned.

Qualifications:

  1. Minimum of three years of experience in direct patient care, preferably in an outpatient setting.
  2. At least two years of experience in Case Management.
  3. Experience in chronic disease management preferred.
  4. Familiarity with infectious diseases and reporting requirements.
  5. Knowledge of quality assurance and regulatory standards.
  6. Strong verbal and written communication skills.
  7. Proficient in Microsoft Office and Electronic Health Records.
  8. Self-motivated with the ability to manage complex tasks.
  9. Detail-oriented with a commitment to confidentiality.
  10. Ability to work independently with minimal supervision.
  11. Bilingual skills are a plus.
  12. Experience with the LGBT community and understanding of relevant issues.
  13. A passion for the organization's mission to improve the lives of LGBT individuals.
  14. Ability to work effectively in a diverse and multicultural environment.

Training and Licensure:

  • Current California RN License
  • BSN or MSN Preferred
  • Current AHA CPR Certification
