Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Open Arms Community Health and Service Center in Los Angeles, CA.

Job Responsibilities
  • Provide comprehensive case management services to individuals and families, including assessments, referrals, advocacy, and coordination of services.
  • Collaborate with a multidisciplinary team to develop and implement individualized care plans, ensuring the delivery of high-quality and culturally sensitive services.
  • Work effectively in a fast-paced and diverse environment, utilizing strong communication and interpersonal skills to build relationships with clients and community partners.
  • Develop and maintain knowledge of community resources and services, including healthcare, mental health, and social services.
  • Utilize strong organizational and time management skills to prioritize tasks and manage multiple cases simultaneously.
  • Proficient in documentation and case management software, with the ability to maintain accurate and up-to-date records.

Requirements
  • Previous experience in case management or social services, with a strong understanding of community resources and services.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
