Targeted Case Manager Supervisor

Location: CHNK Behavioral Health

Position Overview:

The Targeted Case Manager Supervisor is tasked with overseeing the delivery and coordination of Targeted Case Management (TCM) services for clients. This role involves supervising a dedicated team of Targeted Case Managers, ensuring compliance with regulations, and guaranteeing the timely and accurate completion of all necessary documentation, billing, and reporting. The Supervisor will also provide essential training on TCM-related topics to new hires and existing case managers across various programs.

Key Responsibilities

Program Oversight
  • Ensure ongoing quality assurance of documentation to maintain compliance with regulations and timely completion of all reporting and billing requirements.
  • Conduct performance evaluations for staff and implement agency policies and procedures as needed.
  • Manage staff scheduling and service delivery effectively.
  • Facilitate team meetings and biweekly Chronic Co-Occurring Physical Health Conditions (CCPHC) meetings to uphold program fidelity.
  • Ensure staff complete all required regulatory training within the first six months of employment.
  • Oversee the timely submission of service plans, documentation, and billing reports by staff.
  • Assist in the recruitment and hiring process in collaboration with the Director of Outpatient Services.
  • Monitor staff caseloads and address any challenges with the Director of Outpatient Services.
  • Support the development and expansion of TCM services, particularly in building relationships with community partners.
Documentation and Administrative Duties
  • Document reasons for any missed client and family contacts and continue efforts to establish communication.
  • Develop individualized care plans with clients, including periodic revisions to address their needs.
  • Ensure documentation meets contract requirements and adheres to state regulations.
  • Complete discharge plans for clients with three or more service contacts.
  • Follow program operation manuals for documentation timelines.
  • Refer ineligible youth back to the appropriate referral sources with recommendations.
  • Adhere to safety guidelines while providing services and maintain a daily itinerary for accountability.
  • Notify guardians or referral agencies of critical incidents within 24 hours.
  • Participate in supervision sessions with a qualified behavioral health professional at least twice a month.
  • Fulfill administrative requests for client needs, including vouchers and training requests.
  • Attend case consultations and clinical meetings regularly.
Direct Client Services
  • Maintain a caseload of 18-20 clients.
  • Assist clients in completing necessary forms to gather information for care plans.
  • Conduct comprehensive assessments and periodic reassessments as per program guidelines.
  • Help clients and families organize and schedule appointments as outlined in treatment plans.
  • Provide referrals to medical, social, educational, and other services as needed.
  • Coordinate home visits and discharge planning to ensure care plan implementation.
  • Engage with non-recipients related to the client for valuable insights into their needs.
  • Assist with medical needs, including securing necessary approvals for medication changes.
  • Ensure client enrollment in educational programs and attend relevant meetings.
  • Provide clients with crisis contact information and ensure they are aware of emergency procedures.
  • Attend relevant meetings and court hearings concerning assigned clients.
  • Offer transportation for clients to appointments and meetings as required.
Training and Development
  • Deliver training to new staff and across programs as necessary.
  • Possess knowledge of effective coping strategies, crisis intervention, and community resources.
  • Be aware of suicide prevention techniques and screening for risk.
  • Complete case management training within six months of hire and maintain continuing education.
  • Uphold professional boundaries with clients, families, and community partners.
  • Attend mandatory training sessions as assigned.
  • Follow state laws and ethical codes regarding client safety and reporting obligations.
  • Perform essential job functions with or without reasonable accommodation.
  • Other responsibilities as assigned by the supervisor.
Physical Requirements and Work Environment:
  • Regularly required to stand, walk, talk, and hear; use hands for handling objects; and reach with arms.
  • Occasionally required to sit, kneel, squat, and lift objects.
  • Must be able to lift and/or move up to 10 pounds, and occasionally up to 100 pounds for crisis management.
  • Specific vision abilities required include close and distance vision.
  • Work environment noise level is typically moderate.
  • Involves both indoor and outdoor activities with varying temperatures.
  • Cleaning and handling of materials, including chemicals, are part of the job.
