Targeted Case Manager Supervisor

Location: CHNK Behavioral Health

Position Overview:

The Targeted Case Manager Supervisor is accountable for the delivery and coordination of Targeted Case Management (TCM) services to clients. This role involves overseeing a team of professionals (Targeted Case Managers) and ensuring compliance with regulations while facilitating the accurate and timely completion of all necessary documentation, billing, and reporting. The Supervisor will also provide training on relevant topics related to Targeted Case Management for new staff and case managers across various programs.

Key Responsibilities

PROGRAM OVERSIGHT
  • Ensure ongoing quality assurance of documentation to maintain compliance with regulations and timely completion of all required reporting and billing.
  • Conduct performance evaluations for staff and implement agency policies and procedures as needed.
  • Manage staff scheduling and service delivery as required.
  • Facilitate team meetings and biweekly Chronic Co-Occurring Physical Health Conditions (CCPHC) meetings to uphold program fidelity.
  • Verify that staff complete all necessary regulatory training (CORE, SED, SUD, SMI, CCPHC) within the first six months of employment.
  • Ensure timely and accurate submission of service plans, documentation, and billing reports by staff.
  • Assist in the recruitment and hiring of staff in collaboration with the Director of Outpatient Services.
  • Monitor staff caseloads and communicate any challenges to the Director of Outpatient Services as necessary.
  • Support the development and expansion of Targeted Case Management services, particularly in building relationships with community partners.
DOCUMENTATION AND ADMINISTRATIVE DUTIES
  • Document reasons for any missed client and family contacts in the case record and continue efforts to reach out via phone and letter, all of which must be recorded.
  • Develop and periodically revise a specific care plan with the client, outlining goals and actions to address their medical, social, educational, or other service needs.
  • Ensure documentation adheres to contract requirements and state regulations, capturing progress towards goals.
  • Complete discharge plans for all clients with three or more service contacts.
  • Follow timeframes for documentation as specified in the program operations manual.
  • Refer ineligible youth back to the referral source with appropriate recommendations.
  • Adhere to safety guidelines and practices as outlined in the program operations manual, providing a daily itinerary to the supervisor.
  • Notify parents or legal guardians of all critical incidents within 24 hours.
  • Participate in supervision with a qualified behavioral health professional at least twice a month, including individual face-to-face meetings.
  • Ensure client needs are met and follow administrative protocols for requests such as vouchers and training.
  • Attend case consultations and clinical meetings.
DIRECT SERVICE DELIVERY
  • Maintain a caseload of 18-20 clients.
  • Complete necessary releases of information forms with clients and guardians to gather essential information for care planning.
  • Conduct comprehensive assessments and periodic reassessments as per the program operations manual to identify client needs.
  • Assist clients and families in organizing, documenting, and scheduling appointments as outlined in the treatment plan.
  • Provide referrals and related activities to help clients access necessary services.
  • Coordinate home visits, discharge planning, and follow-up activities to ensure care plan implementation.
  • Engage with non-recipients who can provide valuable insights regarding the client's history and current needs.
  • Assist with medical needs, including obtaining guardian approval for medication changes.
  • Facilitate client enrollment in educational programs and attend relevant meetings.
  • Ensure clients have access to crisis contact numbers and emergency information.
  • Attend meetings and court hearings related to assigned clients.
  • Provide transportation for clients to appointments and meetings as required.
TRAINING AND PROFESSIONAL DEVELOPMENT
  • Deliver training to new staff and across programs as necessary.
  • Possess knowledge of effective coping strategies, crisis intervention techniques, motivational interviewing, and community resources.
  • Be knowledgeable about suicide prevention and capable of screening for suicide risk.
  • Complete case management training within six months of hire and maintain continuing education requirements.
  • Maintain professional boundaries with clients, families, and community partners at all times.
  • Attend required training sessions as assigned.
  • Adhere to state laws and professional ethics regarding reporting threats of violence or harm.
  • Be aware of and follow state reporting laws for dependency, neglect, abuse, or human trafficking.
  • Perform essential job functions with or without reasonable accommodation.
  • Complete other duties as assigned by the supervisor.
Physical Requirements and Work Environment:
  • Regularly required to stand, walk, talk, or hear; use hands to handle objects; and reach with hands and arms.
  • Occasionally required to sit, kneel, squat, and lift objects.
  • Must be able to lift and/or move up to 10 pounds, and occasionally lift up to 100 pounds for Safe Crisis Management techniques.
  • Specific vision abilities required include close vision, distance vision, and the ability to adjust focus.
  • The work environment is typically moderate in noise level and includes both indoor and outdoor activities.
  • Cleaning and handling cleaning materials, including chemicals, is part of this position.
