Position Overview:

The Targeted Case Manager Supervisor plays a crucial role in overseeing the delivery and coordination of Targeted Case Management (TCM) services for clients. This position involves supervising a dedicated team of Targeted Case Managers, ensuring compliance with regulations, and guaranteeing the timely and accurate completion of all necessary documentation, billing, and reporting. The Supervisor will also provide essential training on TCM-related topics to new hires and existing case managers across various programs.

Key Responsibilities

Program Oversight
  • Ensure ongoing quality assurance of documentation to maintain compliance with regulations and timely completion of all reporting and billing requirements.
  • Conduct staff performance evaluations and implement agency policies and procedures as needed.
  • Manage staff scheduling and service delivery effectively.
  • Facilitate team meetings and biweekly Chronic Co-Occurring Physical Health Conditions (CCPHC) meetings to uphold program fidelity.
  • Verify that staff complete all necessary regulatory training within the first six months of employment.
  • Ensure timely submission of service plans, documentation, and billing reports by staff.
  • Assist in the recruitment and hiring process in collaboration with the Director of Outpatient Services.
  • Monitor staff caseloads and address any barriers with the Director of Outpatient Services.
  • Support the development and expansion of TCM services, particularly in relation to community partnerships.
Documentation and Administrative Duties
  • Document reasons for any missed client and family contacts and continue efforts to reach out via phone and written communication.
  • Develop and periodically revise individualized care plans with clients, outlining specific goals and actions for addressing their needs.
  • Ensure documentation adheres to contract requirements and state regulations, reflecting client progress towards goals.
  • Complete discharge plans for clients with three or more service contacts.
  • Follow established timeframes for documentation as outlined in the program operations manual.
  • Refer ineligible youth back to the referral source with appropriate recommendations.
  • Adhere to safety guidelines and provide a daily itinerary to supervisors regarding work locations.
  • Notify guardians or referral agencies of critical incidents within 24 hours.
  • Participate in supervision sessions with a qualified behavioral health professional at least twice a month.
  • Ensure client needs are met while following administrative protocols for requests.
  • Attend case consultations and clinical meetings regularly.
Direct Client Services
  • Maintain a caseload of 18-20 clients.
  • Complete necessary releases of information forms to gather essential data for care plan development.
  • Conduct comprehensive assessments and periodic reassessments to identify client needs.
  • Assist clients and families in organizing and scheduling appointments as outlined in treatment plans.
  • Provide referrals and related activities to help clients access necessary services.
  • Coordinate home visits, discharge planning, and follow-up activities to ensure care plan implementation.
  • Engage with non-recipients who can provide valuable insights regarding the client's history and needs.
  • Assist with clients' medical needs, including securing necessary approvals for medication changes.
  • Facilitate client enrollment in educational programs and attend relevant meetings.
  • Ensure clients have access to crisis contact information and emergency services.
  • Attend meetings and court hearings related to assigned caseloads.
  • Provide transportation for clients to appointments and meetings as needed.
Training and Professional Development
  • Deliver training to new staff and across programs as required.
  • Possess knowledge of effective coping strategies, crisis intervention techniques, and community resources.
  • Maintain awareness of suicide prevention practices and screening for risk.
  • Complete case management training within six months of hire and meet continuing education requirements.
  • Uphold professional boundaries with clients, families, and community partners.
  • Attend mandatory training sessions as assigned.
  • Comply with state laws and ethical codes regarding client safety and reporting obligations.
  • Perform essential job functions with or without reasonable accommodation.
  • Other responsibilities as assigned by the supervisor.
Physical Requirements and Work Environment:
  • Regularly required to stand, walk, talk, and hear; use hands for handling objects; and reach with arms.
  • Occasionally required to sit, kneel, squat, and lift objects.
  • Must be able to lift and/or move up to 10 pounds, and occasionally up to 100 pounds for crisis management techniques.
  • Specific vision abilities required include close vision and the ability to adjust focus.
  • Work environment noise level is typically moderate.
  • Involves both indoor and outdoor activities with varying temperatures.
  • Cleaning and handling of cleaning materials, including chemicals, are part of the job requirements.
