Enhabit Home Health & Hospice is looking for a Registered Nurse (RN) or Physical Therapist (PT) to take on the role of Patient Care Transition Specialist.
Key Responsibilities Include:
- Guide patients through the process of navigating post-acute care.
- Evaluate, plan, implement, coordinate, monitor, and assess options and services with the primary aim of ensuring a safe transition from acute care to home health or hospice services.
- Incorporate evidence-based clinical guidelines and preventative protocols in the development of patient-centered transition plans that enhance quality and efficiency in post-acute care delivery.
- Encourage adherence to post-acute plans and ensure that ordered services are fulfilled.
- Act as a representative of Enhabit in transitional care initiatives and build strategic relationships with healthcare systems, hospitals, inpatient facilities, and physician groups.
- Oversee the execution of transitional care services through ongoing quality assurance visits with referral sources.
- Meet and/or exceed referral and admission targets.
- Serve as the clinical liaison responsible for care transitions program admission activities for the designated territory, positively influencing patient outcomes and referral source satisfaction.
The ideal candidate will be a Registered Nurse (RN) or Physical Therapist (PT) who is driven by goals, motivated by sales, and possesses prior experience in home health or hospice settings.
- Must be a graduate of an accredited nursing or therapy program and hold a valid license in the state of employment.
- Minimum of 2-3 years of field experience required.
- Strong understanding of customer and market dynamics, along with transitional care best practices.
- Knowledge of Federal, State, and local laws and regulations governing home health and hospice operations.
- Excellent communication skills with the ability to engage effectively with diverse individuals.
- Experience in territory management, strong presentation skills, performance management, relationship building, negotiation, and results-driven sales planning and execution.
- Self-starter who requires minimal supervision.
- Preferred experience in home health or healthcare sales.
