Key Responsibilities- Provide high-quality wound care services to patients in a home setting, utilizing your expertise in wound assessment, treatment, and management.
- Collaborate with our multidisciplinary team to develop and implement comprehensive treatment plans that address the unique needs of each patient.
- Conduct thorough assessments of patients' medical histories, allergies, symptoms, and current medications to inform treatment decisions.
- Formulate and implement suitable treatment plans, including prescribing medications and providing education to patients and their families.
- Develop and maintain accurate and detailed records of patient interactions, treatment plans, and outcomes.
- Participate in ongoing education and professional development to stay current with best practices in wound care and advance your skills as a Nurse Practitioner.
- Board-Certified Nurse Practitioner (ANP, AGNP, FNP, ENP, or ACNP)
- Masters Degree from an accredited Nurse Practitioner program
- Highly recommended certifications (WCC, CWON, or CWS)
- One year of direct wound care experience
- At least one year experience as a Nurse Practitioner
- Prefer experience in Wound Care, Home Care, Emergency Department or Intensive Care Units
- Must be able to work independently and as part of a team
- Must be willing to learn and demonstrate the ability to communicate effectively with patients, families, and other healthcare professionals
- Competitive salary of $140,000 per year
