Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health LPN - Patient Care Professional. As a LPN, you will visit patients in their homes and provide nursing care for them under the direction of RNs and in compliance with physicians' orders.
Responsibilities- Provide nursing care to patients in their homes, under the direction of RNs and in compliance with physicians' orders.
- Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Administer medications, perform wound care, and provide other nursing interventions as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient care activities, including assessments, interventions, and outcomes, in accordance with company policies and procedures.
- Must possess a valid state driver's license.
- Must possess automobile liability insurance.
- Must wear seat belts at all times while driving.
- Dependable transportation kept in good working condition.
- Must be able to drive an automobile in all types of weather conditions.
- Must be currently licensed as a Licensed Practical Nurse in the State of employment.
