Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health LPN - Patient Care Professional. As a LPN, you will visit patients in their homes and provide nursing care for them under the direction of RNs and in compliance with physicians' orders.

Responsibilities
  • Provide nursing care to patients in their homes, under the direction of RNs and in compliance with physicians' orders.
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient care activities, including assessments, interventions, and outcomes, in accordance with company policies and procedures.
Qualifications
  • Must possess a valid state driver's license.
  • Must possess automobile liability insurance.
  • Must wear seat belts at all times while driving.
  • Dependable transportation kept in good working condition.
  • Must be able to drive an automobile in all types of weather conditions.
  • Must be currently licensed as a Licensed Practical Nurse in the State of employment.
