Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health LPN - Patient Care Professional. As a LPN, you will visit patients in their homes and provide nursing care for them under the direction of RNs and in compliance with physicians' orders.
Responsibilities- Provide high-quality nursing care to patients in their homes, under the direction of RNs and in compliance with physicians' orders.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Collaborate with RNs and other healthcare professionals to ensure seamless care coordination and communication.
- Document patient care activities, including assessments, treatments, and progress notes, in a timely and accurate manner.
- Current licensure as a Licensed Practical Nurse in the state of employment.
- Valid state driver's license and automobile liability insurance.
- Dependable transportation and ability to drive in all types of weather conditions.
- Ability to lift, bend, and move patients as needed.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
