Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Conduct regular home visits to assess patient needs, provide education, and support.
- Document patient progress and communicate with the care team to ensure seamless care coordination.
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- Minimum of one year of recent nursing experience in a behavioral health setting, preferably in home care or a psychiatric hospital.
- Valid CPR certification and a reliable vehicle with insurance.
