We are seeking a skilled Registered Nurse to join our team as a Behavioral Health Home Health Specialist. As a key member of our care team, you will provide high-quality patient care and support to individuals in their homes.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct regular home visits to assess patient needs, provide education, and support.

Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • Minimum of one year of recent nursing experience in a behavioral health setting, preferably in home care or a psychiatric hospital.
  • Valid CPR certification and a reliable vehicle with insurance.
