Hospice Community Outreach Specialist

Tapestry Hospice & Palliative Care is a family-owned and operated business that has been serving North Georgia for over 16 years. We are a care team that values compassion, comfort, and peace for our patients and their families.
We are seeking a skilled professional to serve as a first impression for our organization with referral sources, patients, families, and the community. As a Community Liaison, you will develop and maintain relationships with potential referral sources, including physicians, community health facilities, and other healthcare providers.
Key Responsibilities- Develop and implement a marketing strategy to promote Tapestry Hospice & Palliative Care to potential referral sources
- Build and maintain relationships with referral sources, including physicians, community health facilities, and other healthcare providers
- Identify and pursue new business opportunities to grow our referral base
- Collaborate with our care team to ensure seamless transitions and high-quality patient care
- Participate in ongoing education and training to stay up-to-date on industry developments and best practices
