Position Title: SOAR Coordinator

Reports To: SOAR Manager

Employment Type: Hourly/Non-Exempt

Position Summary:

The SOAR Coordinator plays a crucial role in delivering targeted outreach services to individuals facing mental health challenges and housing instability. This position focuses on screening and assisting clients in applying for disability benefits through the SSI/SSDI Outreach, Access, and Recovery (SOAR) initiative. The coordinator will also engage with hospitals to ensure that inpatients have access to necessary SOAR services.

Key Responsibilities:

  1. Conduct Outreach: Engage with individuals experiencing mental health issues and housing crises to connect them with income, insurance, and essential services.
  2. In-Reach Services: Provide support to inpatients in hospitals, assessing their needs and determining eligibility for SSI/SSDI benefits.
  3. SOAR Process Expertise: Demonstrate knowledge of the SOAR outreach processes, ensuring compliance with program requirements.
  4. Data Collection: Gather all necessary information for the SSI/SSDI application process, including documentation from various sources.
  5. Comprehensive Reporting: Compile clinical data into detailed medical summary reports that outline the individual's disabling condition and its impact on their functionality.
  6. Submission of Evidence: Ensure timely submission of all relevant medical evidence to the Social Security Administration (SSA) and Disability Determination Services (DDS).
  7. Cultural Competence: Collaborate with diverse populations, enhancing cultural understanding and competency within the team.
  8. Community Engagement: Participate in local meetings and advocacy efforts related to homeless services.
  9. Additional Duties: Perform other responsibilities as assigned to support the mission of the organization.

Qualifications:

  • Bachelor's Degree in Psychology, Sociology, Social Work, or a related field.
  • Minimum of two years of experience working with individuals facing mental health challenges, substance use issues, or homelessness.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a team environment.
  • Proficient in time management and meeting deadlines.
  • Valid driver's license and proof of automobile insurance.

Working Conditions:

  • Hybrid work environment, including fieldwork and remote capabilities.
  • Ability to navigate various environments, including potentially challenging outdoor settings.

Physical Requirements:

  • Ability to sit or stand for extended periods.
  • Capable of lifting up to ten pounds and navigating uneven terrain.
