Position Title: SOAR Coordinator

Job Summary:

The SOAR Coordinator plays a vital role in delivering targeted outreach services to individuals facing mental health challenges and housing instability. This position is focused on screening and assisting clients in applying for disability benefits through the SSI/SSDI Outreach, Access, and Recovery (SOAR) Program. The coordinator will also engage with hospital staff to ensure that inpatients have access to SOAR services.

The SOAR initiative is designed to support individuals at risk of homelessness who are dealing with serious mental health issues, medical conditions, or substance use disorders in navigating the Social Security disability benefits application process.

Key Responsibilities:

  1. Outreach Activities: Conduct outreach to individuals experiencing mental health issues and housing crises, facilitating connections to income, insurance, and essential services.
  2. In-Reach Services: Provide in-reach support to hospitalized individuals, assessing their eligibility for SSI/SSDI benefits and offering necessary assistance.
  3. SOAR Process Expertise: Demonstrate proficiency in the SOAR outreach procedures, ensuring all necessary documentation is completed accurately and timely.
  4. Data Collection: Gather and compile all required information for the SSI/SSDI application process, maintaining close communication with local SSA offices.
  5. Medical Summary Reporting: Create comprehensive medical summary reports that detail the history and functional limitations of applicants.
  6. Submission of Evidence: Ensure that all relevant medical documentation is submitted to the SSA/DDS within specified timeframes.
  7. Cultural Competence: Engage with diverse populations, enhancing personal cultural competency through ongoing training and collaboration.
  8. Community Engagement: Attend community meetings and participate in agency activities to promote awareness and advocacy for homeless services.
  9. Additional Duties: Perform other related tasks as assigned to support the overall mission of the organization.

Qualifications:

  • Bachelor's Degree in Psychology, Sociology, Social Work, or a related field.
  • Minimum of two years of experience working with individuals facing mental health challenges, substance use issues, or homelessness.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a team environment.
  • Valid driver's license and proof of automobile insurance.

Working Conditions:
