POSITION OVERVIEW

The Hospice Care Outreach Specialist plays a pivotal role in enhancing hospital-based outreach, managing referrals, and driving the growth of admission rates.

KEY RESPONSIBILITIES
  1. Collaborate with clinical leadership and teams to identify and cultivate potential referral sources, engaging in educational outreach and maintaining professional relationships with healthcare providers to promote hospice services and clarify the hospice benefit.
  2. Exhibit exceptional customer service and critical thinking skills consistently.
  3. Work alongside the medical community, patients, and families to facilitate and encourage eligible hospice admissions.
  4. Partner with referral sources within the hospital to identify and facilitate admissions for patients eligible for GIP level of care.
  5. Oversee patient discharge processes from the hospital and manage admissions to home, inpatient units, or long-term care facilities.
  6. Coordinate the tracking of referral sources, assess the utilization of hospice services, and prepare relationship activity reports.
  7. Manage relationships with skilled nursing facilities and long-term care providers.
  8. Demonstrate the ability to self-direct work with minimal supervision.
  9. Organize educational presentations and in-service training sessions.
  10. Practice effective resource management while considering budget implications.
  11. Maintain a log of referral source expenditures.
  12. Document activities daily within the electronic medical record (EMR) and customer relationship management (CRM) systems.
  13. Conduct on-site assessments to identify needs and provide support to prospective patients, families, and referral sources.
  14. Communicate effectively with physicians, medical directors, and other healthcare providers regarding patient evaluations and care plans.
  15. Complete necessary admission documentation within the scope of practice.
  16. Facilitate appropriate interventions during complex patient referral and admission processes.
  17. Perform other duties as assigned.
SUPERVISORY ROLE

No administrative supervisory responsibilities for agency staff.

PROFESSIONAL DEVELOPMENT

The agency mandates that this position complete 15 hours of continuing education annually on topics that enhance the ability to fulfill the above responsibilities. Regulatory agencies may require additional hours for specific disciplines to maintain licensure or certification.

QUALIFICATIONS

To succeed in this role, candidates must demonstrate the ability to perform essential duties satisfactorily, meeting required competencies. Reasonable accommodations may be provided for individuals with disabilities to perform essential functions.

EDUCATION AND EXPERIENCE

Registered Nurse with a minimum of five years of experience in healthcare, preferably in hospice sales and marketing.

CERTIFICATIONS AND LICENSES

Must possess a current Tennessee or compact RN license, along with a valid Tennessee driver's license and automobile insurance as mandated by state law. This position may involve frequent travel.
