Position Summary

The Hospice Clinical Liaison plays a pivotal role in healthcare outreach, focusing on referral management and enhancing admission rates. This professional is tasked with establishing and nurturing relationships with potential referral sources and healthcare providers to promote hospice services.

Key Responsibilities
  1. Collaborate with clinical leadership to identify and cultivate potential referral sources, ensuring effective education and outreach.
  2. Deliver exceptional customer service and apply critical thinking skills consistently.
  3. Engage with the medical community, patients, and families to facilitate eligible hospice admissions.
  4. Work alongside referral partners to identify and facilitate admissions for patients requiring specialized care.
  5. Oversee patient transitions from hospital settings to home, inpatient units, or long-term care facilities.
  6. Track referral sources and assess the utilization of hospice services, preparing activity reports as needed.
  7. Manage relationships with skilled nursing and long-term care facilities.
  8. Exercise autonomy in work responsibilities with minimal supervision.
  9. Coordinate educational presentations and in-service training sessions.
  10. Demonstrate effective resource management, considering budget implications.
  11. Maintain accurate logs of referral source expenditures.
  12. Document interactions and activities within the electronic medical record (EMR) and customer relationship management (CRM) systems.
  13. Conduct on-site assessments to identify needs and provide support to patients and referral sources.
  14. Communicate effectively with healthcare providers regarding patient evaluations and care plans.
  15. Complete necessary admission documentation within the scope of practice.
  16. Facilitate appropriate interventions during complex referral and admission processes.
  17. Perform additional duties as assigned.
Qualifications

To excel in this role, candidates must meet the following qualifications:

  • Education: Registered Nurse with a minimum of five years of experience in healthcare, preferably in hospice sales and marketing.
  • Licenses: Must possess a current Tennessee or compact RN license and a valid Tennessee driver's license.

Alive Hospice is committed to ongoing professional development, requiring completion of 15 hours of continuing education annually to enhance job performance and compliance with regulatory standards.
